If you have a long-term or permanent medical condition or disability (physical or cognitive), your doctor and / or Transport for NSW may ask for an OT Driving Assessment.


An OT Driving Assessment determines if / how your condition affects your ability to drive safely and legally.


OT Driver Assessors are specially trained to assess:

  • Motor skills, reflexes and response times
  • Memory and judgment
  • Driving behaviour


The goal of the OT Driving Assessment is to enable you to drive safely and legally for as long as possible.


Stage 1 – Pre-drive assessment (Up to 2 hours)

  • Medical and driving history interview
  • Assessment of physical ability, vision, and cognitive skills
  • Basic road rules knowledge


Stage 2 - On-Road Assessment (1 to 1.5 hours)

  • In a dual-control car with an OT and driving instructor
  • Assessment of your real-world driving (observation, planning, reaction time, etc.)
  • If  needed, you may trial vehicle modifications (e.g. hand controls, steering aids)


After the Assessment You’ll get feedback on your driving and how your condition affects it. A report goes to your doctor and TfNSW’s Medical Unit, and the possible outcomes include: 

  • Retain your licence (with or  without restrictions)
  • Downgrade to a learner’s licence for driving lessons or training with modifications
  • Temporary suspension of licence during recovery
  • Medical cancellation of licence


Note:

  • If you require vehicle modifications, you may need lessons and must also pass a TfNSW Disability Driving Test.
  • Reassessments may be needed after lessons or if your condition is progressive (e.g. Parkinson’s, dementia).


  • Valid licence or learner’s permit (not suspended or cancelled)
  • TfNSW Fitness to Drive Medical Report Doctor’s referral outlining the reasons an OT driving assessment
  • Glasses/hearing aids if needed
  • Vision report (if required)


  • You must sign a consent form to allow the report to be shared with your doctor and TfNSW.
  • Without your consent the assessment cannot proceed.


Driving represents freedom and independence, but it’s a privilege, not a right. To keep your licence, you must be medically fit to drive.

  • You must report any long-term or permanent medical condition that may affect your ability to drive to Transport for NSW (TfNSW).
  • Not reporting may put lives at risk and lead to legal or financial consequences, including loss of insurance.


  • Driving involves physical, visual, mental, and emotional skills. 
  • Health changes can affect driving ability, even if you don’t notice them. 
  • It’s important to listen to advice from your doctor and family.
